Report-A-Problem Service Request moving to My Service Center

UT Health San Antonio’s Report-A-Problem Service Request form for Facilities Management will be moving to My Service Center on April 22 to better facilitate online updates and the organization’s growth.

To allow for a seamless transition, Report-A-Problem will have a similar appearance as the current submittal form and will still have the same convenient accessibility from desktops and mobile computing devices.

The Report-A-Problem Service Request allows anyone in the university to report an operations, maintenance or upkeep issue in one of the university’s buildings or grounds. Issues can range from leaking faucets and broken sprinkler heads to out-of-order elevators and temperature-related issues. When a problem is reported, a photo of the issue can be uploaded as an attachment via one’s mobile device.
